We work with almost all major insurance providers, and, if you you have an injury due to an automobile accident, can even bill auto insurance.

Frequently Asked Questions
Do I need to bring anything?
On your first visit, you’ll need to bring the following; your physician referral or prescription (if needed), your ID/drivers license, your insurance card, your primary registration forms, and your co-payment. You may bring a change of clothing, but that is optional. Anything you would exercise in is fine to wear (T-shirt, tank-top, sweatpants, shorts, sneakers).
Do you accept insurance?
We accept most major health insurance plans including Medicare and Worker’s Compensation. Please contact us before your first visit to check if we accept your specific health plan.
What can Physical Therapy Treat?
Some of the common treatments we do include: back and neck pain, Arthritis, Osteoporosis, Pelvic Dysfunction, elbow, shoulder and hand pain, Work Conditioning and Industrial Rehab, Vestibular Repetitive stress injuries and sports injuries.
Do you accept Worker's Compensation/ Rehabilitation cases?
Yes, we do! We offer Work Rehabilitation, Post-Offer Employment Testing, Functional Capacity Evaluations, Job Demands Analysis & Pre-Work Screens and Educational Classes.
Do you work on Lien?
Yes, we perform services on a lien basis (we treat you now and get paid later).
Do I need a doctor's referral to receive physical therapy?
If your insurance plan allows self-referral to physical therapy, a physician referral is not necessary. However, please check with your insurance, since many health insurance plans require one. We also offer services that do NOT require a referral through a discounted cash plan.
How long is each visit?
Each visit will last about 60 – 90 minutes. Timing depends upon the treatment and exercises being performed.
How can I make an appointment?
Call 678-505-0000 to request an appointment. If you have a physician’s referral, please bring it to your first appointment. If you don’t have a referral, we will work with your physician to obtain one.
